Please fill out this form if your club is requesting more than $500 in funding. This form MUST be filled out in addition to the Budget Request Form on Club Macaulay, or else your proposal will not be considered. This form must also be filled out by that month's budgetary deadline to be considered for funding that month (please see the list of deadlines).
